Identification

• Citizen Service Number and Identification Obligation
Since 1 June 2009, all healthcare providers, indication bodies and health insurers must use the citizen service number (BSN) when exchanging data about patients or clients. This means that all these parties must work with a reliable BSN. They must be sure that the number and details belong to a specific person.
• Clients must be able to identify themselves at a hospital and other care providers, such as the general practitioner, dietician, physiotherapist or nursing home. This means that you must have proof of identity with you when you visit my practice.

Prevention

If you are unable to come, you can report this by telephone or e-mail at least 24 hours in advance. The consultation will then not be charged. If you do not show up for an appointment without a timely notification, the consultation will not be reimbursed by the insurer and I am forced to charge you for the appointment time with a minimum of € 20.00.

Terms and Conditions

General
1. In these General Terms and Conditions, the following definitions apply:
Dietitian: professional who provides nutritional information and dietary advice and who is authorized to use the title of dietician according to the Individual Healthcare Professions Act.
Client/Patient: natural person who consults the dietician for nutritional information or dietary advice.
Diet: food on medical indication.
Dietary advice: advice regarding a diet.
Nutritional advice: advice on healthy nutrition without a medical indication.
2. The patient or client or his/her legal representative can purchase the services of a dietician after the nature of the service has been agreed in an intake interview: dietary advice or nutritional advice.
3. In the case of dietary advice, the counseling of the patient cannot start before the patient can submit a referral from a (general) doctor.
4. The dietician informs the patient or client about the costs of the dietary advice.
5. This time consists of the time the patient has spent in the consultation room with the dietitian (direct patient-related time) and the time the dietitian has spent on patient-related activities where the patient is not present (indirect patient-related time).
6. The patient or client undertakes to reimburse fees from the first consultation.
7. The dietician gives the invoice to the client or the client receives the invoice digitally. The patient must pay a bill to the dietician within the set term of 10  to dawn. The patient can then send the bill to the health insurance company, whereby the health insurer refunds part of the amount to the patient. The practice Eet & Beleef has no contracts with health insurers.
8. Non-reimbursement or partial reimbursement of the costs of dietary advice by the health insurer, whereby the patient is insured, does not suspend the payment obligation.
9. If the patient or client cancels an appointment made with a dietician no earlier than 24 hours before that appointment, the dietician may charge the intended consultation to that patient or client. The bill amounts to a maximum of four times the quarter-hourly rate.
10. The patient or client may terminate the agreement at any time, provided that the dietitian has been notified of this in writing 24 hours prior to an appointment made.
11. The dietitian may terminate the agreement if the advice of the dietician does not lead to the desired results. The dietitian will inform the patient or client in good time if the dietitian sees such a situation coming.
12. Articles 4 to 10 of the General Terms and Conditions also apply to nutritional advice.
13. The privacy of the client is guaranteed. Everything discussed with the client will be treated confidentially. In accordance with standard practice, the dietitian will report to the physician/referrer on the progress and content of the treatment, unless the client requests in writing not to do so. The dietician uses Siilo to send information securely by e-mail.
14. The dietician's advice is by nature result-oriented without guaranteeing that result.
15. The dietician excludes any liability with regard to damage or injury arising from or related to the follow-up by the client of the advice provided by the dietitian, unless there is intent or gross negligence on the part of the dietitian.
16. The dietician excludes any liability with regard to injury or damage arising from or related to the Basic Health Check or following a diet.

17. The dietitian will inform the referring physician of the measurements of the Basic Health Check after approval by the client.

Payment Terms Dietitian

1. The amounts declared by the dietitian to a patient or his/her legal representative for fees, costs and other reimbursements arising from the consultation must be submitted within 10  days after the date stated on the invoice.
2. If the patient or client does not pay the amount due within 10  days after the date stated on the invoice, the patient or client is in default, without further notice of default being required. The patient or client owes the dietitian statutory interest on the amount due from the day on which he/she is in default, as long as the patient or client fails to meet his/her obligations. The dietitian will claim that interest if the patient or client has not fulfilled his/her obligations even after the term referred to in Article 3.
3. If, in the case referred to in Article 2, the patient or client has not yet fulfilled his/her obligations after a payment reminder within the period of 2 weeks stated therein, the dietitian is entitled to immediately proceed to collect the claim, then to charge third parties with this.
4. All costs involved in the collection of invoiced amounts (including the extrajudicial collection costs) will be borne by the patient or client. The extrajudicial collection costs amount to a minimum of € 40.00.
5. Unless the contrary is expressly proven, the dietician's administrative data is binding with regard to the correctness of what is owed by the patient or client.
6. Complaints that cannot be resolved mutually can be submitted to the Paramedics First Line Complaints Committee, PO Box 1161, 3800 BD Amersfoort.
7. A copy of the general and payment conditions is available free of charge. Ask the dietitian about it.
